Appointments
For your ease and convenience, we schedule appointments at three locations throughout south Denver. All appointments may be scheduled through our main Parker office by calling 720-851-2000, Monday – Thursday, 9:00 a.m - 5:00 p.m. and Friday, 9:00 a.m. - 4:00 p.m. One of our staff members will be happy to help you.

New Patient Forms
Once you've scheduled an appointment, please download and complete the following forms in advance and bring them with you when you register on the day of your appointment:

Privacy Policy Form
Informed Consent & Pain Management Policy Form

Patient Financial and Payment Policy Form

Other items to bring on your visit include:

• All x-rays, MRI scans, CT scans or myelograms
• Previous spine-related medical records
• Physician referral (if required by your insurance provider)
• Insurance card
• Insurance co-payment

Any medical records brought or sent to our office become part of your permanent patient file. If you need copies, please make them prior to releasing them to us.

If your visit is job- or accident-related, please also provide date of injury/accident, worker’s comp or auto insurance company, phone and policy numbers.

If you are a patient with a Medical Lien, you and your attorney must
sign and fax back the following form before coming to our offices:
